Family Support services

Case Management

Family Support Specialists work with families to create obtainable goals (3-6 month time frame). The specialist will work to connect families to resources and assist with completing applications and other necessary paperwork. The goal is to empower and strengthen the family. Please complete this form to make a referral to the Family Support Case Management Program.

How we help you

Whole Village uses the National Standards of Quality for Family Strengthening and Support. Using family voice to help guide and engage others in our work. These programs are designed to empower families on their journey of raising children. Each program has a specific goal and outcome to engage and ensure successful modeling to individual families while also building natural support in their local communities.

Newborn navigation

This is a home visiting program under the Comprehensive Family Supports and Services contract with the Department of Health and Human Services. Up to three home visits are offered to families.

All expectant and new parents residing in Belknap and Lower Grafton Counties are eligible to be connected with a trained Newborn Navigator from the Family Resource Center.

The program can start prenatally or any time within the first 3 months of the baby’s birth. Visits are scheduled at the family’s convenience.

A referral is sent to Carol Stone, at the Family Resource Center. She will contact the family and the first visit will be scheduled.
